How does this work?
step 1 : getting a referral from your GP
Ask your GP if a review of your medicines would be helpful for you. If your GP thinks it will help, they will send a referral to a credentialed pharmacist.
step 2: Making appointment
I will contact you for the home visit. Get all the medicines you are taking, including puffers, vitamins or supplements.
step 3: Home visit
I will visit you at your home to review your medications and address any questions you may have. After the visit, I will prepare a detailed report for your GP. You are welcome to have a family member or carer present during the visit, and they can also ask questions.
step 4 : GP review
See your GP again and they will talk with you about a plan for your medicines.
